All of the Services that we offer are available for people with a disability through the NDIS

In our social support program our team of dedicated and experienced Disability Support Workers provide assistance to adults with a disability to access and participate in activities of their own choice.

Our aim is to provide person-centred and meaningful programs which assist clients as much or as little as necessary in order to stay independent.

Who is it for?

Our Social Support programs cater for adults with acquired and inherited disability and low to medium support needs.

Disability Support Workers provide the following services:

  • Comprehensive needs assessment
  • Individual and goal-oriented planning
  • Plan evaluations
  • One to one or group support
  • Hands-on assistance while accessing community and taking part in activities
  • Motivation and encouragement
  • Liaison with families and professionals
  • Referrals and follow-ups

What we offer:

  • Group programs:
    • Chatswood Social Club
    • Theatre Ensemble “Different Degrees”
    • Music Therapy
    • Outings and trips
  • Individual programs/ cluster of supports
    • Community access
    • Recreation and leisure activities e.g. accessing gym, places of interest, concerts
    • Daily life skills e.g. budgeting, cooking, healthy lifestyle
    • Travel assistance and travel training

We are registered as a service provider with NDIS.
