Our free Community Advisory Service welcomes enquiries about community supports and services that can enable people to stay living at home, independently and more confidently.

Our experienced advisor can listen to your concerns, answer your questions about individual circumstances and if required, help you access the following:

  • Commonwealth Home Support Program services, such as cleaning, transport, home delivered meals, nursing, etc
  • My Aged Care – the Federal Government’s pathway for Commonwealth Home Support Program services, Home Care Packages, respite and permanent residential care
  • Sydney Community Services programs including our social support, activities and events
  • NSW Health’s aged care and allied health services

Everyone’s circumstances are different but common concerns often include falls, memory, family stress, social isolation and just generally finding day-to-day tasks more difficult.

Federally subsidised Commonwealth Home Support Program services are available for eligible people aged over 65 years, or over 50 years if identifying as an Aboriginal or Torres Strait Islander.

The service also connects carers to the NSW Carers Gateway. A free telephone interpreting service is available through the Translating & Interpreting service (TIS National) on 131 450.

Feeling informed about what’s available and having guidance on how to move forward with a plan of action leads to greater optimism about living at home, both now and in the future.
