Sydney Community Services provides nursing and personal care services to eligible veterans, war widow/widower partners, and people holding a DVA Gold Card or White Card.
Our experienced Registered Nurses and Support Workers deliver professional, compassionate care in the comfort of your own home, helping you maintain your health, independence and wellbeing.
We work closely with you, your family, your GP and other healthcare professionals to ensure you receive coordinated care that meets your individual needs.

Nursing Services
Our Registered Nurses can provide a range of clinical services, including:
- Medication support and management
- Post-hospital discharge care
- Short-term clinical care
- Continence assessment and advice
- Insulin administration
- Diabetes monitoring
- Wound care and pressure injury
- management
- Skin care and monitoring
Personal Care Services
Our Support Workers can assist with everyday personal care tasks such as:
- Showering and personal hygiene
- Dressing and undressing
- Grooming and presentation
Who Is Eligible?
You may be eligible for DVA-funded nursing or personal care services if you are:
- A veteran holding a DVA Gold Card
- A veteran holding a DVA White Card (for approved conditions)
- A war widow or widower partner receiving DVA support

How to access this service
Step 1
Speak with your GP.
Your GP can assess your needs and provide a referral for DVA-funded nursing services.

Contact us and speak with one of our Coordinators. Learn about the cost involved and the funding option that best suits you.
